The Congregational Health Index or “CHI” is a survey tool and guide to help your congregation discover opportunities for improving your health, and challenges to getting there. The CHI will show you how to make healthy eating and physical activity choices available to everyone at your faith community.

Why Our health is powerfully shaped by the places we work, learn, play and worship. Our daily environments make healthful diets and physical activity easier or harder to achieve. In recent decades, these environments have changed drastically, making it more challenging to maintain a healthy way of life in many communities. When fast food options are cheap and convenient, but healthy foods are not, and neighborhoods lack safe areas for outdoor play, our health suffers. With the right tools, we can change our immediate environments to make healthy choices the easy choices.
